PS Personal Statement is required CV CV is required RL Three letters of recommendation Transcript Arrange to have official transcripts mailed from each postsecondary institution attended (not needed if only attended UF) and be sure to include your UF ID # on the sealed envelopes. Students applying from another state university in Florida should request from their Registrar's Office that the transcript be sent electronically. International applicants must mail an official copy in the native language of each transcript or mark sheet, along with official English translations for transcripts not originally issued in English. In addition, they must mail a final attested copy of the diploma/degree certificate from the registrar or comptroller of each university or college attended, along with official English translations for diplomas not originally issued in English. We do not accept: Provisional degree certificates Emailed, faxed, photocopied or uploaded degree certificates and transcripts Degree certificates and transcripts notarized by a public notary Degree certificates and transcripts not in an envelope sealed by the academic institution If you have questions regarding international transcripts, please send an email.
