PS Provide an original and personal Statement of Purpose. You will upload this document with the online application under section A of "Upload Documents". Include the following details within the Statement of Purpose:?? If submitting a check or money order, include your full legal name and date of birth. RL Provide three letters of recommendation. The letters should accurately reflect your academic aptitude, work ethic, knowledge and character. Transcript We require one copy of the scanned transcript from each undergraduate and graduate institution which you attended. If you have earned a Bachelor's or Master's degree, please include a copy of your degree certificate. Unofficial transcripts must be submitted from each institution, regardless of the length of attendance, and whether or not courses were completed.
